In accordance with this Disclosure Statement, your personal data is processed by Op. Dr. Fatih TURAN ENT Clinic in the capacity of Data Controller, and always in compliance with the Law No. 6698 on the Protection of Personal Data (“Law” or “KVKK”).
Your personal data may be processed by the Data Controller for the following purposes:
To fulfill contractual and legal obligations accurately and completely;
To retain required personal data under applicable legislation;
To assert and defend legal rights;
To protect the activities, assets, privacy, and security of the clinic, its business partners, yourself, or others;
To apply remedies and minimize harm when necessary.
Identity Information: Name, surname, national ID number, mother’s name, father’s name, place and date of birth, nationality.
Employment and Education Information: Details collected through physical applications, including employment status, contact details, education level (e.g., high school, university, postgraduate), past graduation data, attended courses/seminars, certifications, and national/international exam results.
Contact Information: Residential address, workplace address, phone number, and email address.
Financial Data: Bank account number, IBAN, credit/bank card information, banking details.
Signature Data: Signatures on contracts and official documentation.
Health Data: Medical reports, disability status, blood type, and health information required for occupational health and safety purposes.
HR Records: Payroll records, disciplinary records, employment start forms, résumés, and performance evaluation reports.
Your personal data may be shared, within the scope of the above purposes and as required, with domestic service providers and business partners (such as lawyers, financial advisors), and legally authorized public institutions (e.g., SGK, İŞKUR) and/or private individuals. Transfers will be made in line with the conditions and purposes outlined in Articles 8 and 9 of the Law.
Your personal data is collected by the Data Controller through various channels such as interviews, job listing platforms, recruitment agencies, HR consultancies, or directly via résumé submission. Collection and processing may take place in physical or electronic formats, either manually or via automated systems forming part of a data recording system.
The primary legal basis for processing is to evaluate your job application and potentially establish an employment relationship. Accordingly, your personal data may be processed under the following provisions of the Law:
Article 5(2)(c): Necessary for the establishment or performance of a contract,
Article 5(2)(e): Necessary for the establishment, exercise, or protection of a legal right,
Article 5(2)(d): If your personal data has been made public by you,
Article 5(2)(f): If processing is necessary for the legitimate interests of the Data Controller, provided it does not harm your fundamental rights and freedoms.
In cases where none of these conditions apply, your personal data will only be processed based on your explicit consent in accordance with Article 5(1) and Article 6(2).
As the owner of personal data, you are entitled to:
Learn whether your personal data has been processed;
Request information if your personal data has been processed;
Learn the purpose of processing and whether your data is used in accordance with that purpose;
Know the third parties to whom your personal data has been transferred domestically or internationally;
Request correction of your personal data if it is incomplete or inaccurate, and request notification to third parties of such corrections;
Request deletion or destruction of your personal data under the terms set out in the Law, and request notification to third parties of such actions;
Object to any unfavorable result arising from analysis carried out solely through automated systems;
Request compensation if you suffer damage due to unlawful data processing.
You may submit your requests regarding these rights by filling out the KVKK Application Form available at our clinic or by contacting us via the email address provided in your CV. Your application will be processed and responded to within 30 (thirty) days at the latest, depending on the nature of the request. If the fulfillment of your request incurs a cost, a fee may be charged in accordance with the tariff determined by the Personal Data Protection Board.
